The Ohio EFDA Registration Process |

The Ohio State Dental
Board has initiated the Ohio EFDA Registration process
as of 2007. The EFDA registration application is
currently downloadable on their website. |

Current Ohio EFDAs, and
those who have just passed the EFDA state exam, are
required to send the Ohio State Dental Board proof of
successful completion of EFDA training, proof of passing
the CODT state EFDA examination, current CPR
certification, and HBV immunization along with $20 and a
recent color photograph. |
